Request an Accommodation

Please send this form to the LTB as soon as possible so that the LTB has enough time to consider your request before the hearing. You may send the form by email, mail, fax or courier.
For more information, please see the Request an Accommodation webpage.
Request a Mediation or Hearing in French
There are two ways to request a mediation or hearing in French:
- Complete the Request for French-Language Services included in the application form; or
- Complete the Request for French-Language Services and email it to the regional office that is handling your file.
A bilingual dispute resolution officer or adjudicator will be assigned to your mediation or hearing. Make your request as soon as possible and well in advance of your mediation or hearing date.
